Best Camera (recently reviewed here), just updated to offer Flickr upload support. Best Camera was already my choice for photo sharing app on the iPhone because of its ability to upload to both Facebook and Twitter at the same time. Adding Flickr support adds icing to the cake for an already great social networking feature set. Not only can you upload to Flickr, but you can also choose which Photo Set (album) to upload to. You can even create new Photo Sets on the fly. As with the Facebook and Twitter support you can add a title to the photo and description before uploading. You can also utilize Flickr's privacy controls in marking a photo for viewing by the general Public, Friends, Family, Friends and Family or Private.
